Q. I can only select a certain number of checkboxes in a section.

A. Check the section description for select limitations. It may only allow a certain number of selections.

Q. I do not see any way to enter data into the ratings section.

A. Please ensure that you have selected a provider in section 3.
For 'Other' systems, please enter the appropriate text in the given field after clicking the checkbox. Then click the plus button ( [+] ) to add it to section 5.

Q. The online form does not seems to be working properly, in general.

A. Please check that both cookies and javascript are enabled in your browser, using the following URL:
